Donald Jeffrey Requate, 47, of Chetek, Wisconsin passed away on Monday, October 1, 2012 at the Mayo Clinic Health System Northland in Barron, Wisconsin. He was born on March 18, 1965 in Miami, Florida the son of Ferdinand Requate and Faith (Lakeman) Bierbaum.

Private family services will be held at a later date.

The Burnham Ours Funeral Home of Chetek, Wisconsin has been entrusted with arrangements.
